Payment Policy:

Counseling and therapy services are provided directly to you and payment is made directly to me at the time of service. If you wish to submit to your insurance company for reimbursement, I provide you with a detailed billable form with all of the required information on it. Once you have given me your insurance information, these forms (if requested) will be given to you at each session. You can attach the billable form to your insurance form to submit for reimbursement. You may wish to call your insurance company prior to treatment to determine your out of network mental health benefits. I will fill out a uniform treatment plan for you if that is deemed necessary. However, I do not make these arrangement with your insurance company. You will need to provide me with the information necessary to support you in receiving your insurance benefits, including dates when you need another plan for treatment (if required).

Health Insurance:

I believe that your therapy belongs to YOU, not your insurance company. Thus, I do not participate in managed care plans nor do I accept assignment of benefits, which means that I do not accept insurance for counseling or therapy sessions. I am an "out-of-network" provider; your insurance company will not pay me directly. You may be reimbursed by your insurance company after direct payment has been made for services provided.

Will you get reimbursed for Counseling or Therapy Sessions?

I am a Ph.D. (Doctor), with an LCPC (Licensed Clinical Professional Counselor) and NCC (Nationally Certified Counselor), and my professional services qualify for patient reimbursement under most insurance plans. My fees are usually considered to be reasonable and customary, within the acceptable range (UCR), by most insurance companies. Psychotherapeutic services are usually covered by most insurance plans but you will have to contact them to find out directly.
